Responsible as a member of the home health team too provide medical social services to the patient and family in the home. This will be consistent with Harmony Home Health & Hospice policies and procedures. Qualifications · Graduate of a school of social work approved by the Council of Social Work Education with a master’s degree · Prior home health experience preferred but not necessary · Demonstrate experience in working with patient/families · Licensure by the state, if applicable · A criminal background check must be performed before hire · Complete and pass skill evaluation test Job Duties Provide supportive and/or counsel to patients/families in keeping with patients/families needs. This includes assisting patients and their families to understand, accept and follow medical recommendations Complete a psychosocial assessment with the initial social work referral and make appropriate recommendations regarding the plan of care.
